RBC Casts

RBC casts, or red blood cell casts, are cylindrical structures composed of red blood cells that are found in the urine. They are formed in the distal convoluted tubule or the collecting duct of the nephron, when the tubular fluid becomes concentrated and acidic, causing the cells to stick together and form a cast in the shape of the tubule. The presence of RBC casts in urine is a significant diagnostic marker, primarily indicating glomerulonephritis, which is an inflammation of the glomeruli in the kidney. This condition can lead to hematuria (blood in the urine), which is a common symptom associated with kidney diseases.

Formation and Significance[edit | edit source]

RBC casts are formed under conditions where the kidney's filtering system is damaged. This damage allows red blood cells to leak from the glomeruli into the tubules. In the presence of a high salt concentration and low pH, these cells can adhere to one another and to Tamm-Horsfall protein, a mucoprotein secreted by tubule cells, forming a cast. The presence of these casts in the urine, known as hematuria, is a hallmark of glomerular disease and can be indicative of acute or chronic glomerulonephritis, vasculitis, or malignant hypertension.

Diagnosis[edit | edit source]

The detection of RBC casts is primarily done through a urine test, specifically a microscopic examination of urinary sediment. This test is part of a urinalysis, which may also measure other aspects of urine composition. The identification of RBC casts requires careful microscopic examination, as they can be easily overlooked or confused with other types of casts or debris.

The presence of RBC casts in urine is a critical diagnostic finding that requires further investigation to determine the underlying cause. It is most commonly associated with glomerulonephritis, but can also indicate other kidney diseases or conditions that cause glomerular damage. Treatment and prognosis depend on the underlying condition, but the detection of RBC casts in urine is a sign that immediate medical attention is needed.
