Hawk-i

Hawk-i is a health insurance program for children who live in the state of Iowa. It is designed to provide coverage for children in families with incomes too high to qualify for Medicaid, but too low to afford private insurance. The program is funded by both the state and federal governments.

Eligibility[edit | edit source]

To be eligible for Hawk-i, a child must be under the age of 19 and not eligible for Medicaid. The child must also be a resident of Iowa and a U.S. citizen, U.S. national, or qualified alien. The child's family income must be at or below 300% of the federal poverty level.

Hawk-i covers a variety of health care services, including:

  • Doctor visits
  • Hospital care
  • Prescriptions
  • Dental care
  • Vision care
  • Mental health services
  • Preventive care

Families can apply for Hawk-i online, by mail, or by phone. The application is available in English and Spanish. There is no cost to apply.

The cost of Hawk-i depends on the family's income. Some families may not have to pay anything, while others may have to pay a monthly premium. There are no co-pays for any services.
