History of present illness, as the name suggests, is a detailed description of the onset, course, and any associated symptoms, risk factors, etc that relevant to the patients current illness. It is preceded by the chief complaint in a typical history taking of a case.

HPI[edit | edit source]

Abbreviated as HPI, it is a chronological description of the development of the patient's present illness from the first sign and/or symptom or from the previous encounter to the present.

Elements of HPI[edit | edit source]

It includes the following elements:

  • location;
  • quality;
  • severity;
  • duration;
  • timing;
  • context;
  • modifying factors; and
  • associated signs and symptoms.

Types - Brief and extended HPI[edit | edit source]

Brief and extended HPIs are distinguished by the amount of detail needed to accurately characterize the clinical problem(s).

Brief HPI[edit | edit source]

  • A brief HPI consists of one to three elements of the HPI.
  • The medical record should describe one to three elements of the present illness (HPI).

Extended HPI[edit | edit source]

  • An extended HPI consists of four or more elements of the HPI.
  • The medical record should describe four or more elements of the present illness (HPI) or associated comorbidities.
