Moorfields Eye Hospital NHS Foundation Trust is an NHS foundation trust that operates Moorfields Eye Hospital. It was established on 21 March 1994. The trust employs over 2,400 staff and provides services to over 300,000 patients annually. It operates from its main site on City Road in London, as well as over 20 satellite clinics located in various parts of London and the South East.

Moorfields Eye Hospital NHS Foundation Trust is one of the first NHS foundation trusts and is a founding member of UCLPartners, an academic health science centre. The trust also offers commercial services, including private care, corporate venturing, and pharmaceutical manufacturing.

The trust has plans to relocate its main hospital from City Road to St Pancras Hospital in Camden. The new hospital is expected to be operational by 2025–26 and will cost £352 million.

In addition to its services in the UK, Moorfields Eye Hospital NHS Foundation Trust opened a branch in Dubai, UAE in 2007. The trust is also exploring international healthcare accreditation.

The trust requires full funding agreements for all treatments in advance of accepting NHS referrals from outside England. In 2015, it was owed over £600,000 for treatments provided in the previous three years.
