How can Sabizabulin help in COVID-19?

  • A new phase 3 clinical study of Sabizabulin was stopped due to overwhelming positive results of reduced death in COVID-19 patients.
  • This global study conducted in many countries including the United States, Brazil, Colombia, Argentina, Mexico, and Bulgaria.
  • It studies the effect of the drug on patients who were hospitalised with the Delta and Omicron variants.
  • In total, there were 150 patients in the study, with half given sabizabulin and the other half remaining on placebo.
  • Sabizabulin reduced deaths by 55%, showing clear efficacy
  • Both the treatment group and control group were allowed what is considered standard COVID care, including remdesivir, dexamethasone, anti-IL6 receptor antibodies, and JAK inhibitors.
  • The group that received COVID treatment, Sabizabulin, reduced deaths at the day 60 point by an immense 55%.
  • The placebo group had a mortality rate of 45%, while the treatment group had a 20% mortality rate.

How does Sabizabulin work?

  • Upon oral administration, sabizabulin binds to the colchicine-binding site of alpha- and beta-tubulin subunits of microtubules and crosslinks the microtubules, thereby inhibiting microtubule polymerization in tumor (or virus infected) blood vessel endothelial cells and tumor cells.
  • This blocks the formation of the mitotic spindle and leads to cell cycle arrest at the G2/M phase. As a result, this agent disrupts the tumor vasculature, tumor blood flow, deprives tumor cells of nutrients, and induces apoptosis.
  • Also, as microtubules plays an important role in intracellular transport, the inhibition of its polymerization may disrupt the transport of the androgen receptor (AR) into the cell nucleus, as well as virus trafficking around the cell which may decrease viral replication and assembly.
  • Inhibition of tubulin polymerization may also inhibit the release of pro-inflammatory cytokines and disrupt inflammatory cell activities.
  • Sabizabulin is not a substrate of P-glycoprotein (Pgp), an efflux pump that when overexpressed, may confer resistance to taxane agents.
