Biliary hamartomas

Biliary hamartomas, also known as von Meyenburg complexes, are benign liver lesions that are considered to be a congenital malformation of the bile ducts. These lesions are typically small, often less than 1.5 cm in diameter, and are composed of irregularly shaped bile ducts embedded in a fibrous stroma. Biliary hamartomas are usually discovered incidentally during imaging studies for unrelated reasons, as they rarely cause symptoms or require treatment.

Etiology and Pathogenesis[edit | edit source]

The exact cause of biliary hamartomas is not well understood, but they are believed to be congenital, arising from an aberrant development of the intrahepatic bile ducts during embryogenesis. This developmental anomaly results in the formation of cystic spaces that are lined by biliary epithelium and surrounded by a fibrous stroma, which do not communicate with the biliary tree.

Clinical Presentation[edit | edit source]

Most individuals with biliary hamartomas are asymptomatic, and the lesions are often discovered incidentally during imaging studies such as ultrasound, CT scan, or magnetic resonance imaging (MRI). On rare occasions, if the hamartomas are large or numerous, they may cause vague abdominal discomfort or pain.

Diagnosis[edit | edit source]

The diagnosis of biliary hamartomas is primarily radiological, with characteristic appearances on ultrasound, CT scan, and MRI. On ultrasound, they typically appear as small, hypoechoic lesions scattered throughout the liver. CT and MRI provide more detailed images, showing well-defined, non-enhancing lesions that do not communicate with the biliary tree. In some cases, a liver biopsy may be performed to confirm the diagnosis, showing the characteristic histopathological features of biliary hamartomas.

Treatment and Prognosis[edit | edit source]

No specific treatment is required for biliary hamartomas, as they are benign and usually asymptomatic. Management is generally conservative, focusing on monitoring for any potential complications, although these are rare. The prognosis for individuals with biliary hamartomas is excellent, with no known association with liver dysfunction or malignant transformation.

Differential Diagnosis[edit | edit source]

The differential diagnosis for biliary hamartomas includes other liver lesions such as simple liver cysts, hepatic adenomas, focal nodular hyperplasia, and hepatocellular carcinoma. Imaging studies, along with clinical correlation and sometimes biopsy, are essential for accurate diagnosis and differentiation from these other entities.
