Bohler's angle

Bohler's Angle is a radiographic measurement used in the field of orthopedics to assess the severity of calcaneal fractures. Named after Lorenz Böhler, an Austrian surgeon who was a pioneer in the field of trauma surgery, Bohler's angle is an important diagnostic tool in the management of calcaneus fractures.

Definition[edit | edit source]

Bohler's angle, also known as the calcaneal angle or the tuber-joint angle, is formed by two lines drawn on a lateral radiograph of the foot. The first line is drawn from the highest point of the anterior process of the calcaneus to the highest point of the posterior articular facet. The second line is drawn from the highest point of the posterior articular facet to the highest point of the calcaneal tuberosity. The angle formed by the intersection of these two lines is Bohler's angle.

Clinical Significance[edit | edit source]

Bohler's angle is used to assess the severity of calcaneal fractures. A normal Bohler's angle ranges from 20 to 40 degrees. A decrease in Bohler's angle is indicative of a calcaneal fracture. The degree of decrease in the angle can also provide information about the severity of the fracture.

Measurement[edit | edit source]

To measure Bohler's angle, a lateral radiograph of the foot is required. The patient should be in a supine position with the foot in a neutral position. The X-ray beam should be directed perpendicular to the film.

Limitations[edit | edit source]

While Bohler's angle is a useful tool in the diagnosis and management of calcaneal fractures, it has its limitations. It is dependent on the quality of the radiograph and the positioning of the foot. It is also subject to interobserver variability.
