CYP27B1 is an enzyme that plays a crucial role in the metabolism of vitamin D. It is also known as 25-hydroxyvitamin D3 1-alpha-hydroxylase. This enzyme is responsible for the hydroxylation of 25-hydroxyvitamin D3 (calcidiol) into 1,25-dihydroxyvitamin D3 (calcitriol), the active form of vitamin D. The conversion process is essential for calcium absorption and bone health, as well as for the modulation of the immune system.

CYP27B1 is located in the mitochondria of kidney cells, where it catalyzes the conversion of calcidiol to calcitriol. Patients with VDDR-1 are unable to convert calcidiol to calcitriol, leading to low levels of calcium in the blood, impaired bone mineralization, and rickets.
