Esuberaprost is a synthetic analog of prostacyclin, a potent vasodilator and inhibitor of platelet aggregation. It is primarily used in the treatment of pulmonary arterial hypertension (PAH) and other conditions characterized by poor blood flow.

Pharmacology[edit | edit source]

Esuberaprost acts by mimicking the effects of prostacyclin, a naturally occurring prostaglandin that plays a critical role in vascular homeostasis. Prostacyclin binds to the prostacyclin receptor (IP receptor) on the surface of endothelial cells, leading to increased levels of cyclic adenosine monophosphate (cAMP) within the cell. This results in vasodilation and inhibition of platelet aggregation, which are beneficial in conditions like PAH where blood flow is restricted.

Clinical Use[edit | edit source]

Esuberaprost is administered orally and is used in the management of pulmonary arterial hypertension. It helps to improve exercise capacity and delay clinical worsening in patients with PAH. The drug is often used in combination with other PAH therapies, such as endothelin receptor antagonists and phosphodiesterase-5 inhibitors.

Side Effects[edit | edit source]

Common side effects of esuberaprost include headache, flushing, jaw pain, and gastrointestinal disturbances. These side effects are generally mild and tend to decrease with continued use. However, patients should be monitored for signs of hypotension and bleeding due to the drug's vasodilatory and antiplatelet effects.

Mechanism of Action[edit | edit source]

Esuberaprost exerts its effects by binding to the IP receptor, which is a G protein-coupled receptor. Activation of this receptor leads to the activation of adenylate cyclase, increasing the production of cAMP. Elevated cAMP levels result in the relaxation of vascular smooth muscle cells and inhibition of platelet aggregation.
