Health assessment

Health assessment is a systematic method of collecting and analyzing data about a patient's health status. It includes a comprehensive evaluation of the physical, psychological, social, and environmental aspects of an individual's health. The purpose of a health assessment is to identify health problems, determine the risk for future health problems, provide a baseline for future assessments, and establish a relationship between the patient and the healthcare provider.

A health assessment typically includes a medical history, a physical examination, and sometimes laboratory tests or other diagnostic procedures. The information gathered during a health assessment is used to develop a nursing care plan or a medical treatment plan.

It includes information about the patient's symptoms, medical conditions, medications, allergies, family history, and lifestyle. The medical history helps the healthcare provider to understand the patient's health status and to identify any potential health risks.

The physical examination involves a systematic examination of the patient's body. It includes an assessment of the patient's vital signs, general appearance, and the function of various body systems. The physical examination provides objective data about the patient's health status.

Laboratory Tests and Diagnostic Procedures[edit | edit source]

Laboratory tests and diagnostic procedures may be used to confirm or rule out a suspected diagnosis, to monitor the progress of a disease, or to evaluate the effectiveness of a treatment. They provide objective data that can be used to make informed decisions about the patient's care.

Nursing Care Plan or Medical Treatment Plan[edit | edit source]

Based on the findings of the health assessment, the healthcare provider develops a nursing care plan or a medical treatment plan. This plan outlines the strategies and interventions that will be used to address the patient's health problems and to promote the patient's health and well-being.
