Hebeloma crustuliniforme is a species of mushroom in the genus Hebeloma. It is commonly known as the poisonpie or fairy cake mushroom. This fungus is widely distributed in North America and Europe, where it is found in forests, woodlands, and grassy areas.

Hebeloma crustuliniforme has a convex to flat cap that ranges in color from pale yellow to brown. The cap surface is smooth and often sticky when moist. The gills are adnate to sinuate and are initially white, becoming pinkish-brown as the mushroom matures. The stem is cylindrical and often has a swollen base. The flesh is white and has a mealy or farinaceous odor.

Hebeloma crustuliniforme is considered toxic and should not be consumed. It contains toxins that can cause gastrointestinal upset and other symptoms if ingested. It is important to properly identify mushrooms before consuming them to avoid accidental poisoning.
