The assessment should be based on the healthcare professional's expertise and the information available at the time.

The Recommendation component of SBAR is the healthcare professional's recommendation for action. This could include a proposed treatment plan, a request for a consultation, or a recommendation for further testing. The recommendation should be clear, concise, and actionable.
