Urinary cast[edit | edit source]

Urinary cast is a tube-shaped protein structure that can be found in the urine. They are formed in the distal convoluted tubule and collecting ducts of the nephrons in the kidneys. The presence of urinary casts can indicate certain medical conditions, such as kidney disease.

Formation[edit | edit source]

Urinary casts are formed when the protein Tamm-Horsfall secreted by the renal tubular cells, comes in contact with the acidic urine. The protein precipitates and forms a cast, which takes the shape of the tubule where it was formed. The casts are then flushed out of the kidney with urine.

Types[edit | edit source]

There are several types of urinary casts, each indicating a different condition. These include:

  • Hyaline casts: These are the most common type of urinary cast and are composed of a specific type of protein known as Tamm-Horsfall protein. They can be seen in healthy individuals and are usually not a cause for concern.
  • Red blood cell casts: These casts are indicative of glomerulonephritis, a type of kidney disease. They are formed when red blood cells leak into the urine.
  • White blood cell casts: These casts are indicative of inflammation or infection in the kidney. They are formed when white blood cells leak into the urine.
  • Granular casts: These casts are indicative of acute tubular necrosis, a condition where the tubular cells of the kidney are damaged.
  • Waxy casts: These casts are indicative of chronic kidney disease. They are formed when the granular casts stay in the kidney for a long time and become waxy.

The presence of urinary casts is usually detected during a routine urine test, known as a urinalysis. The type of cast present can help in diagnosing the underlying condition.

Treatment[edit | edit source]

The treatment for urinary casts depends on the underlying condition that is causing them. This can range from medication to manage symptoms, to more intensive treatments such as dialysis or kidney transplant for severe kidney diseases.
