BI-RADS

BI-RADS (Breast Imaging-Reporting and Data System) is a standardized system developed by the American College of Radiology (ACR) to categorize and report findings in breast imaging. The system is designed to improve communication between radiologists and referring physicians, ensure consistent reporting, and facilitate research and data collection.

History

The BI-RADS system was first introduced in 1993 and has undergone several revisions to incorporate advances in breast imaging technology and clinical practice. The most recent edition, BI-RADS 5th Edition, was published in 2013.

BI-RADS categorizes breast imaging findings into seven categories, each with specific management recommendations:

• BI-RADS 0: Incomplete

 - Additional imaging evaluation and/or comparison to prior mammograms is needed.

• BI-RADS 1: Negative

 - No significant findings; routine screening recommended.

• BI-RADS 2: Benign

 - Non-cancerous findings; routine screening recommended.

• BI-RADS 3: Probably Benign

 - Findings have a high probability of being benign; short-interval follow-up suggested.

• BI-RADS 4: Suspicious

 - Findings are suspicious for malignancy; biopsy should be considered. This category is further divided into:
   - 4A: Low suspicion for malignancy
   - 4B: Moderate suspicion for malignancy
   - 4C: High suspicion for malignancy

• BI-RADS 5: Highly Suggestive of Malignancy

 - Findings have a high probability of being cancer; appropriate action should be taken.

• BI-RADS 6: Known Biopsy-Proven Malignancy

 - Lesions confirmed to be malignant by biopsy; appropriate action should be taken.
