Hand-Assisted Laparoscopy is a surgical technique that combines traditional laparoscopy and open surgery. This technique allows the surgeon to introduce their hand into the patient's abdomen while maintaining the pneumoperitoneum necessary for laparoscopic surgery.

History[edit | edit source]

The technique of hand-assisted laparoscopy was developed in the 1990s as a way to combine the benefits of both open and laparoscopic surgery. It was initially used in gastrointestinal surgery, but has since been applied to a wide range of surgical procedures.

Technique[edit | edit source]

In hand-assisted laparoscopy, the surgeon makes a small incision in the patient's abdomen, through which a special device known as a hand-port or gelport is inserted. This device allows the surgeon to introduce their hand into the patient's abdomen while maintaining the pneumoperitoneum necessary for laparoscopic surgery. The surgeon can then use their hand to manipulate organs and tissues, while using laparoscopic instruments in the other hand to perform the surgery.

Advantages[edit | edit source]

Hand-assisted laparoscopy has several advantages over traditional laparoscopic surgery. It allows for tactile feedback, which can be particularly useful in complex or difficult surgeries. It also allows for quicker and safer extraction of specimens, and can reduce the overall operative time.

Disadvantages[edit | edit source]

The main disadvantage of hand-assisted laparoscopy is that it requires a larger incision than traditional laparoscopic surgery, which can lead to increased postoperative pain and a longer recovery time. However, these disadvantages are often offset by the benefits of the technique.

Applications[edit | edit source]

Hand-assisted laparoscopy is used in a wide range of surgical procedures, including colectomy, nephrectomy, and splenectomy. It is also used in bariatric surgery, liver resection, and adrenalectomy.
