If you have a claim in process, your health plan or health insurance company may ask for a letter of medical need.
Letter from healthcare provider[edit | edit source]
This means they need your health care provider to show that you needed their services.
Purpose[edit | edit source]
The letter has to provide roof the treatment or service is medically necessary.
