The history of the hospital dates back to 1910 when it was known as Altamont Hospital in Christiansburg. It started with 12 beds and was later expanded to 20 beds. In 1967, it was reorganized into a community hospital and renamed as the A.M. Showalter Memorial Hospital. In 1969, Hospital Corporation of America provided financing for the construction of the current facility. Over the years, the hospital went through several name changes and transfers of ownership. In 2010, it was renamed LewisGale Hospital Montgomery as part of HCA's LewisGale Regional Health System.

In 2006, the hospital was the site of a tragic shooting incident. William Morva, who was awaiting trial on an attempted robbery charge, shot and killed a security guard in the emergency department.
