The company was established in 1961 by Ken Selby, a former Marine Corps member, in Tulsa, Oklahoma. Ken Selby, a former Marine and school teacher, opened the first Mazzio's restaurant, originally named The Pizza Parlor, in 1961. The restaurant was renamed to Mazzio's in 1975. The name "Mazzio's" is derived from the Italian word "mazzo", which means "bundle" or "heap", reflecting the restaurant's generous portion sizes.

Mazzio's menu primarily consists of pizza, but also includes pasta dishes, sandwiches, and salads. The pizza offerings range from traditional favorites like pepperoni and sausage to more unique options like taco pizza and barbecue chicken pizza.
