Adrenal androgen

Adrenal androgens are steroid hormones produced by the adrenal glands that function as weak androgens (male sex hormones). These hormones are produced in both males and females and play a key role in the development of male traits and reproductive activity. The most well-known adrenal androgens include dehydroepiandrosterone (DHEA), dehydroepiandrosterone sulfate (DHEAS), and androstenedione.

Production and Secretion[edit | edit source]

Adrenal androgens are synthesized in the zona reticularis, the innermost layer of the adrenal cortex. Their production is stimulated by the adrenocorticotropic hormone (ACTH) released from the pituitary gland. Unlike other steroid hormones produced by the adrenal glands, such as cortisol, adrenal androgens are not considered essential for life. However, they play significant roles in the body, especially during development and puberty.

Functions[edit | edit source]

Adrenal androgens serve as precursors to more potent androgens like testosterone and dihydrotestosterone (DHT), as well as to estrogens in both men and women. In prepubertal children, adrenal androgens contribute to the development of pubic and axillary hair, known as adrenarche. They also influence libido, bone density, and muscle strength. In women, adrenal androgens can affect the menstrual cycle and fertility.

Clinical Significance[edit | edit source]

      1. Hyperandrogenism

An excess of adrenal androgens can lead to conditions characterized by hyperandrogenism, such as congenital adrenal hyperplasia (CAH) and polycystic ovary syndrome (PCOS) in women. Symptoms may include hirsutism (excessive hair growth), acne, menstrual irregularities, and infertility.

      1. Adrenal Tumors

Adrenal tumors, including adrenocortical carcinoma and benign adenomas, can also lead to increased production of adrenal androgens, resulting in similar symptoms of hyperandrogenism.

      1. Addison's Disease

Conversely, Addison's disease, a condition characterized by insufficient production of adrenal hormones, including androgens, can lead to a decrease in secondary sexual characteristics and libido.

Diagnosis and Treatment[edit | edit source]

The diagnosis of disorders related to adrenal androgen excess or deficiency involves measuring the levels of these hormones in the blood or urine. Treatment depends on the underlying cause but may include glucocorticoids to suppress ACTH production and reduce androgen secretion, anti-androgens, or hormone replacement therapy.
