The brain and sleep[edit]
Sleep is an active biological process[edit]
During sleep, the brain continues processing information and repeatedly moves through stages of non-rapid eye movement sleep and rapid eye movement sleep.
Sleep supports:
- Memory consolidation
- Learning
- Immune function
- Hormone regulation
- Metabolism
- Emotional regulation
- Tissue maintenance and recovery
Sleep is regulated partly by accumulated sleep pressure and partly by the body’s circadian rhythm.[1]
Curious? The sleeping brain reduces its response to many sounds but can remain sensitive to meaningful signals such as an alarm, a baby crying, or a person’s name.
Genes influence sleep need[edit]
People do not all require precisely the same amount or timing of sleep. Age, health, behavior, environment, and genetics all contribute.
Researchers have identified genes associated with:
- Sleep duration
- Morning or evening preference
- Insomnia
- Some circadian-rhythm disorders
- Certain rare sleep conditions
Feeling accustomed to insufficient sleep does not prove that chronic sleep deprivation is harmless.[1]
The body contains biological clocks[edit]
The central circadian clock is located in the suprachiasmatic nucleus of the hypothalamus. Light detected by the retina helps synchronize this clock with the day–night cycle.
Circadian rhythms influence:
- Sleep and alertness
- Body temperature
- Melatonin production
- Cortisol secretion
- Digestion
- Metabolism
- Some immune functions
Travel across time zones can separate the internal clock from local time, causing jet lag.
The immune system can attack the nervous system[edit]
Multiple sclerosis is an autoimmune disease in which the immune system attacks components of myelin, the protective material surrounding many nerve fibers.
Damage to myelin can disrupt communication between the central nervous system and the rest of the body. Symptoms vary because different areas of the brain and spinal cord may be affected.[2]
The microbiome[edit]
The body is not an isolated organism. It is also a habitat for complex communities of microorganisms.
The human body is an ecosystem[edit]
The human microbiome includes communities of bacteria, viruses, fungi, and archaea living on and within the body.
Important microbial habitats include:
- The gastrointestinal tract
- Skin
- Oral cavity
- Nose and respiratory tract
- Reproductive tract
Hundreds of bacterial species may be present in a person at a given time. The precise composition differs among individuals and across body sites.[3]
Key idea: Not every microorganism is harmful. Many coexist without causing disease, while some participate in digestion, metabolism, and immune development.
The intestine and brain communicate in both directions[edit]
The gut–brain axis connects the gastrointestinal system, microbiome, immune system, endocrine system, and nervous system.
Signals may travel through:
- The vagus nerve
- Immune mediators
- Hormones
- Microbial metabolites
- Circulating metabolic signals
Research suggests that the microbiome may influence digestive function, stress responses, and brain biology. However, many proposed microbiome therapies remain experimental.[4]
Evidence check: Association does not prove causation. Claims that one food or probiotic can “reset” the microbiome or cure many unrelated conditions usually exceed current evidence.
Genes and modern medicine[edit]
CRISPR has entered clinical medicine[edit]
In December 2023, the United States Food and Drug Administration approved Casgevy, the first FDA-approved treatment using CRISPR/Cas9 genome-editing technology.
For eligible patients with sickle cell disease, blood-forming stem cells are collected and edited to increase production of fetal hemoglobin. The modified cells are later returned to the patient.[5]
Treatment also requires intensive conditioning and specialized hematopoietic stem cell transplantation. It is not a routine injection or simple office procedure.
Gene therapy does not always edit DNA directly[edit]
Gene therapy may work by:
- Adding a functional gene
- Altering gene activity
- Silencing a harmful gene
- Editing a specific DNA sequence
- Modifying cells outside the body
- Delivering genetic material into the body
Some therapies use modified viral vectors as delivery vehicles. Others use a patient’s own cells.
Gene therapy is therefore a family of technologies—not a single treatment.
One altered gene can affect the entire body[edit]
Sickle cell disease results from a change affecting hemoglobin, the oxygen-carrying protein inside red blood cells.
The molecular change can alter red-cell shape and blood flow, potentially causing:
This demonstrates how a change at the DNA level can influence proteins, cells, circulation, organs, and whole-body health.
The heart and circulation[edit]
The heart is a coordinated system of pumps and valves[edit]
The human heart contains four chambers and four principal heart valves. The valves open and close in sequence to maintain forward blood circulation.
A valve may become:
- Too narrow—stenosis
- Unable to close fully—regurgitation
- Infected—infective endocarditis
- Damaged by congenital disease, aging, or inflammation
Valve disease may progress quietly before producing obvious symptoms. The aortic valve is the valve most commonly affected by disease in the United States.[6]
A childhood throat infection can damage the heart years later[edit]
In susceptible people, infection with group A Streptococcus can trigger rheumatic fever. The immune response may then inflame and scar the heart valves, producing rheumatic heart disease.
Appropriate diagnosis and antibiotic treatment of streptococcal infection can help prevent rheumatic fever.[7]
Curious? The original infection may be gone long before the consequences of the immune reaction become apparent.

Cardiovascular disease, type 2 diabetes, stroke, chronic kidney disease, metabolic dysfunction-associated steatotic liver disease, and some cancers share modifiable risk factors.
These include:
- Tobacco use
- Hypertension
- Physical inactivity
- Unhealthy dietary patterns
- Harmful alcohol consumption
- Abnormal blood lipids
- Inadequately controlled diabetes
- Inadequate sleep
Noncommunicable diseases account for more than three-quarters of deaths globally.[8]
Immunity and infection[edit]
Microorganisms—not patients—become resistant to antibiotics[edit]
Antimicrobial resistance occurs when microorganisms develop or acquire characteristics that allow them to survive antimicrobial treatment.
Resistance can affect:
Antibiotics do not treat viral illnesses such as the common cold or influenza. Unnecessary antibiotic exposure can increase selection pressure favoring resistant bacteria.[9]
Sepsis involves a dangerous response to infection[edit]
Sepsis is a medical emergency involving organ dysfunction caused by a dysregulated response to infection.
Warning signs can include:
- Confusion or altered mental status
- Difficulty breathing
- Rapid heart rate
- Fever, shivering, or feeling unusually cold
- Severe pain or discomfort
- Low urine output
- Mottled or clammy skin
Antimicrobial resistance can make the underlying infection more difficult to treat.[10]
Vaccines can prevent certain cancers[edit]
Persistent infection with certain types of human papillomavirus can cause cervical cancer and contribute to several other cancers.
HPV vaccination is most effective when administered before exposure to the virus. The hepatitis B vaccine can also reduce the risk of liver cancer by preventing chronic hepatitis B infection.[11]
Wellness facts[edit]
Wellness is more than the absence of disease. It includes physical, mental, emotional, social, and environmental factors that help people function and live meaningfully.
Physical activity can provide benefits before body weight changes[edit]
The health effects of physical activity are not limited to weight loss. A single session may improve short-term feelings of anxiety, while regular activity can support:
- Cardiovascular health
- Insulin sensitivity
- Sleep quality
- Bone and muscle health
- Mobility and balance
- Mood and cognitive function
Adults are generally advised to obtain at least 150 minutes of moderate-intensity activity weekly, or an equivalent amount of vigorous activity, plus muscle-strengthening activity.[12]
Wellness takeaway: Activity does not have to occur in a gym. Walking, cycling, dancing, gardening, swimming, household activity, and active transportation can all contribute.
Sitting less can be a meaningful first step[edit]
People who are currently inactive do not have to reach a perfect exercise target immediately. Replacing some sedentary behavior with light or moderate movement can be a realistic beginning.
Helpful strategies include:
- Brief walking breaks
- Taking stairs when practical
- Standing during selected tasks
- Walking while making telephone calls
- Gradually increasing duration or intensity
- Adding resistance training when appropriate
People with significant medical conditions, symptoms, or mobility limitations may need individualized guidance.
A healthy dietary pattern matters more than a single “superfood”[edit]
A health-supporting diet generally emphasizes:
- Vegetables
- Fruit
- Legumes
- Whole grains
- Nuts and seeds
- Appropriate sources of protein
- Mostly unsaturated fats
It generally limits excess sodium, free sugars, trans fats, and highly processed foods with poor nutritional profiles.[13]
Frozen and canned fruits and vegetables can remain nutritious choices when they do not contain excessive added sugar or sodium.
Myth check: No single food can compensate for an otherwise consistently unhealthy dietary pattern.
Fruit juice is not nutritionally identical to whole fruit[edit]
Whole fruit generally provides intact structure and dietary fiber. Many fruit juices contain substantial amounts of free sugars and can be consumed more rapidly than whole fruit.
This does not mean that every person must completely avoid juice. It means portion, dietary context, metabolic health, and overall eating pattern matter.[14]
Social connection is a health factor[edit]
Meaningful social connection can support:
- Stress management
- Healthy behavior
- Emotional well-being
- Sleep quality
- Resilience during illness
- A longer and healthier life
Social isolation and loneliness are associated with increased risks of depression, anxiety, cardiovascular disease, cognitive decline, and earlier death.[15]
Curious? Wellness can be influenced not only by individual choices but also by whether communities provide safe public spaces, transportation, libraries, parks, and opportunities to connect.
Mental health is part of overall health[edit]
Mental health includes emotional, psychological, and social well-being. It affects coping, relationships, learning, work, decision-making, and physical health.
Persistent distress is not a personal failure. Mental-health conditions may involve biological, psychological, social, environmental, and economic factors.
Evidence-based help may include:
- Psychological therapy
- Social support
- Lifestyle interventions
- Medication when appropriate
- Treatment of contributing medical conditions
- Crisis and community services
Seeking help is a health action, not a sign of weakness.
Oral health is connected to general health[edit]
Oral health affects eating, speaking, comfort, confidence, and quality of life. Oral diseases share several risk factors with other noncommunicable diseases, including:
- High intake of free sugars
- Tobacco use
- Harmful alcohol consumption
- Limited access to preventive care
- Poor oral hygiene
Severe periodontal disease is estimated to affect more than one billion people worldwide.[16]
Regular oral hygiene and appropriate dental care are parts of preventive healthcare.
Tobacco affects nearly every organ[edit]
Tobacco exposure is associated with cancer, coronary artery disease, stroke, chronic lung disease, reduced fertility, impaired immunity, and premature death.
Second-hand smoke also harms people who do not use tobacco. The World Health Organization states that there is no safe level of tobacco exposure.[17]
Stopping tobacco use can provide health benefits even after years of exposure. Evidence-based cessation support may include counseling, medication, and structured follow-up.
Sustainable habits often matter more than dramatic short-term efforts[edit]
Long-term health behavior is influenced by:
- Environment
- Cost and accessibility
- Culture
- Sleep and stress
- Social support
- Work schedules
- Mental health
- Medical conditions
- Realistic goal setting
A modest habit that can be continued may produce more lasting value than an extreme plan followed briefly.
Wellness should not be treated as moral perfection. Health opportunities and outcomes are also affected by the social determinants of health.
Medical advances[edit]
A weight-management medicine became an approved treatment for sleep apnea[edit]
In December 2024, the FDA approved tirzepatide, marketed as Zepbound, for moderate-to-severe obstructive sleep apnea in adults with obesity, together with a reduced-calorie diet and increased physical activity.
It was the first medication specifically approved in the United States for this indication.[18]
It does not eliminate the need for appropriate sleep testing or individualized treatment. Some patients may still require continuous positive airway pressure or other therapies.
Some treatments are made from a patient’s own cells[edit]
In several modern therapies, clinicians collect a patient’s cells, modify or select them in a laboratory, and return them to the patient.
Examples include:
- CAR T-cell therapy
- Autologous stem cell transplantation
- Some forms of gene therapy
- Experimental immune-cell therapies
These treatments blur the traditional boundaries between a medicine, a transplant, and a personalized biological product.
Medical knowledge is expected to change[edit]
Recommendations may change when researchers obtain:
- Larger studies
- Longer follow-up
- More representative participants
- Better measurements
- Evidence of rare adverse effects
- Improved statistical methods
- Independent confirmation
An evidence-based revision does not necessarily mean the earlier science was fraudulent. Updating conclusions in response to better evidence is part of the self-correcting nature of science.

Test your curiosity[edit]
Which becomes resistant to an antibiotic—the patient or the microorganism?
Answer: The microorganism. Resistant organisms survive exposure and can reproduce or transfer resistance genes.
Can a vaccine prevent cancer?
Answer: Yes. HPV and hepatitis B vaccines prevent infections responsible for several cancers.
Does being physically fit prevent altitude sickness?
Answer: No. Rate of ascent and adequate acclimatization remain important even for trained athletes.
Is wellness determined only by personal choices?
Answer: No. Income, education, housing, food access, neighborhood safety, discrimination, healthcare access, and other social determinants also influence health.
How to evaluate a medical claim[edit]
- Identify the source. Is the author or organization clearly named?
- Look for evidence. Is the claim supported by a study, guideline, or regulatory document?
- Check the study type. Findings in cells or animals may not produce an effective human treatment.
- Examine the number of participants. Very small studies are more vulnerable to chance and bias.
- Look for a comparison group. Improvement alone does not establish that a treatment caused it.
- Check for independent confirmation.
- Look for risks and limitations. Promotional claims often emphasize benefits while omitting harms.
- Identify conflicts of interest. Is the source selling the proposed treatment?
- Read beyond the headline. Headlines may exaggerate research findings.
- Check the date. Medical evidence and regulatory approvals can change.
Useful principle: The more extraordinary the medical claim, the stronger and more independently verified the evidence should be.
