WBC cast refers to a structure found in the urine during microscopic examination, specifically in the sediment. WBC casts are cylindrical structures produced by the kidney and are composed of white blood cells (WBCs). Their presence in urine is a significant indicator of renal pathology, particularly indicating inflammation or infection within the kidneys, such as pyelonephritis or glomerulonephritis. Understanding WBC casts and their implications in medical diagnosis is crucial for healthcare professionals.

WBC casts are formed in the distal convoluted tubule or the collecting duct of the nephron, the functional unit of the kidney. When there is inflammation or infection in the kidney, white blood cells are recruited to the site to combat the infection. Some of these cells may enter the tubular system of the kidney. In the presence of a protein matrix, typically Tamm-Horsfall protein, secreted by tubular cells, these WBCs can become entrapped and form a cast. The presence of these casts in urine is an abnormal finding, reflecting pathology within the kidney itself.

The detection of WBC casts in urine is a diagnostic hallmark of intrinsic kidney disease, particularly of the inflammatory or infectious type. Their presence differentiates upper urinary tract infections, such as pyelonephritis, from lower tract infections, like cystitis. WBC casts indicate that the kidney's filtration system is involved, suggesting a more severe condition than infections limited to the bladder or urethra.

  • Pyelonephritis: An infection of the kidney typically caused by bacteria that ascend from the lower urinary tract. WBC casts are often present in the urine of affected individuals.
  • Glomerulonephritis: A group of diseases that injure the part of the kidney that filters blood (glomeruli). WBC casts may be seen along with red blood cell casts.
  • Interstitial nephritis: An inflammation of the kidney's tubules and surrounding structures. WBC casts can be present, indicating the inflammatory nature of the disease.

The presence of WBC casts is detected through a urinalysis, specifically microscopic examination of urinary sediment. This test is often part of a routine examination or may be ordered when kidney infection or damage is suspected. The identification of WBC casts requires a trained eye, as they can be easily overlooked or confused with other types of casts.

Treatment and Prognosis[edit | edit source]

The treatment of conditions associated with WBC casts involves addressing the underlying cause. In the case of bacterial infections, antibiotics are the mainstay of therapy. For inflammatory conditions, corticosteroids or other immunosuppressive agents may be required. The prognosis depends on the severity of the underlying condition and the timeliness of treatment initiation.
