Ramsay Hunt syndrome (RHS), also known as Herpes zoster oticus, is a rare neurological disorder characterized by paralysis of the facial nerve (also known as the seventh cranial nerve) and a rash affecting the ear or mouth. The syndrome is named after the British neurologist James Ramsay Hunt, who first described it in 1907.

Etiology

Ramsay Hunt syndrome is caused by the Varicella zoster virus (VZV), the same virus that causes chickenpox in children and shingles in adults. After a person recovers from chickenpox, the virus can remain dormant in the body's nerve cells and reactivate years later, causing Ramsay Hunt syndrome.

Symptoms

The primary symptoms of Ramsay Hunt syndrome are sudden facial weakness or paralysis and a painful rash on the ear (herpes zoster oticus) or in the mouth. Other symptoms may include tinnitus (ringing in the ears), hearing loss, vertigo (dizziness), nausea and vomiting, and difficulty speaking or swallowing.

Diagnosis

Diagnosis of Ramsay Hunt syndrome is based on the characteristic signs and symptoms, particularly the presence of the rash and facial paralysis. Tests such as MRI scans or lumbar puncture may be used to rule out other conditions.

Treatment

Treatment for Ramsay Hunt syndrome typically involves antiviral medications to combat the Varicella zoster virus, along with corticosteroids to reduce inflammation and swelling. Pain medications, physical therapy, and sometimes surgery may also be part of the treatment plan.

Prognosis

The prognosis for Ramsay Hunt syndrome varies. Some people recover completely, while others may have long-term complications such as permanent facial weakness or hearing loss.
